I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity. In consideration of my application and permitting me to participate in this activity, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:
I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this activity, THE FOLLOWING ENTITIES OR PERSONS: Radhika Lakhani and/or their directors, officers, employees, volunteers, representatives, agents, and the activity holders, sponsors, and volunteers ("Retreat Host").
I agree to indemnify Melia Punta Cana Beach ("Venue") and the Retreat Host for any costs, losses, or damage directly or indirectly caused by my actions during my stay.
This Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
MEDICALLY AND MENTALLY FIT
I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity.
I understand that photos and video may be taken at the event, for the attendees keepsake, as well as for future events (we will use our discretion of course!).
I CERTIFY that I am physically fit and have not been advised to not participate by a qualified medical professional. I CERTIFY that there are no health-related reasons or problems which preclude my participation in this activity.
I understand that yoga classes, like any physical activity, present a risk of injury. I take full responsibility for any injuries you may sustain or damages you may incur by participating in this retreat.
By booking this retreat, I affirm that I am well enough to attend this retreat and am able to safely perform moderate meditation, breath work and yoga exercises. I also undertake to share any relevant health information with the Retreat Host in a timely and clear manner.
I will not attend the retreat if I test positive for Covid or are showing symptoms, and shall refer to Ontario’s Public Health Measures for more information https://www.ontario.ca/page/public-health-measures-and-advice.
LOCATION RELATED SAFETY
I understand that the venue is located at an all-inclusive resort with indoor and outdoor facilities. I take full responsibility for any injuries I may sustain or damages I may incur on the property, including but not limited to the time I spend indoors and outdoors .
ALLERGIES & DIETARY RESTRICTIONS
I undertake to share dietary allergies and restrictions in a timely manner so that the kitchen may accommodate my needs. I understand that if I do not share this information when it is requested that the venue and retreat host may not be able to accommodate my restrictions.
RETREAT RULES & RESTRICTIONS
I understand that this Retreat is designed to be a safe & inclusive venue for transformation and agree to comply with the following rules:
- I understand and agree that there is no alcohol or recreational drugs are to be brought or consumed at this event, for the wellness of all attendees including myself, and can result in being removed from the event on the discretion of the named organizers herein.
- Open flames, including candles, and incense, are not permitted anywhere on the property.
- Firearms are prohibited.
- Fireworks and hazardous materials shall not be used on the property.
- Occupancy shall be peaceful and not disturb the neighbours’ right to privacy and the quiet enjoyment of the properties.
- Harassment of any kind is unwelcome and will not be tolerated.
I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.
COMPLETION OF THIS REGISTRATION SHALL BE CONSTRUED AS MY ELECTRONIC SIGNATURE OF THIS RELEASE AND WAIVER.
